When we take your reservation, we will need a lot of information
from you. We want to be sure that we get to know your pet personally, so he
can receive the best possible care during his stay. The following checklist
can help you prepare, so you will have all the necessary information ready at the
time of your reservation:

Read our Boarding Requirements below. It will outline
the vaccinations required as well as some of our policies regarding boarding. |

Be sure your pet is current on his vaccinations.
This is important for the health of your pet as well as the health of all our guests. |

Leave a phone number where we can reach you while you are
away, as well as local emergency contact information. |
|
Tell us about any health concerns or special needs your
pet may have, i.e. old age, blindness or if your dog is fearful of strangers, or
a biter. The better we understand the needs of your pet, the more pleasant
and comfortable his stay will be. |

Tell us what extra activities you would like your pet to participate in during his
stay. |

Vaccinations
We must have proof
of the following vaccinations prior to boarding. Your vet may fax
the records to us at (480) 855-6963.
DOGS:
Distemper/Parvo Combination - current within 1 year
Rabies For Puppies - current within 1 year
Rabies For Adults - current within 3 years
Bordatella —current according to your vet’s protocol
CATS:
FVRCP – current within 1 year
Rabies For Kittens - current within 1 year
Rabies For Adults - current within 3 years

Flea and Tick
Treatment
All dogs must receive
a flea and tick treatment upon arrival unless they are on
a monthly treatment (Frontline, Advantix, etc.) at home. This is for the protection of all
our guests. Bark Avenue's Flea and Tick treatment fee is $12.00. The
fee for this treatment is waived if your pet receives a grooming service during
his stay. |

Feeding
For dogs we use a chicken
and rice diet from California Natural, mixed with canned meat. For Cats, we feed a chicken
and rice diet from Natural Choice. If you wish,
you may bring your pet’s regular food, in which case we ask that it be in portion-sized
packages for each feeding.

Bedding
We provide a freshly-laundered fleece mat to each dog each evening. Dog cots
are provided in suites. For an additional charge, we will provide a cot in our standard
rooms.. Please do not
bring blankets or dog beds as they present hygienic
challenges.

Toys
Guests are allowed 2 toys each, which must be waterproof
and larger than a tennis ball. Each toy must by clearly marked with your pets
name in permanent ink. Please, no rawhide chews toys. |
